Pemfigus vulgaris adalah suatu penyakit akibat dari kelainan autoimun yang berupa vesikel atau bulla pada kulit ataupun mukosa yang berasal dari lapisan suprabasal epidermis yang dihasilkan dari produksi autoantibodi terhadap desmoglein 1 dan 3. Biasanya, lepuh muncul pada kulit yang tampak normal, tetapi dapat berkembang pada kulit yang eritematosa. Karena lepuh pemfigus vulgaris rapuh, lesi kulit yang paling umum diamati pada pasien adalah erosi akibat lepuh pecah. Penegakan diagnosis dari pemfigus vulgaris dapat ditegakkan jika ditemukan hasil positif pada pemeriksaan klinis berupa tanda nikolsy sign positif, pemeriksaan histologi, dan uji imunologik, atau dua tanda yang mengarah diagnosis pemfigus vulgaris dan adanya uji imunologik). Tatalaksana pemfigus vulgaris dibagi dalam 3 fase, yaitu fase kontrol, fase konsolidasi, dan fase maintenance yang terfokus pada pemberian kortikosteroid jenis prednisone yang diberikan sesuai dosis tertentu dengan monitoring ketat agar mencegah dari efek samping sistemik yang dapat terjadi akibat pemberian kortikosteroid jangka panjang. Pada pasien hipertensi dengan pemfigus vulgaris, maka dosis metilprednisolon yang diberikan ialah 40-125 mg dengan dosis terbagi. Pemfigus Vulgaris dapat disebabkan oleh stres oksidatif yang dalam jumlah berlebihan ke tingkat yang lebih besar dari kapasitas proses antioksidan tubuh dapat menyebabkan apoptosis sel dan akumulasi sisa-sisa apoptosis, mendorong pembentukan autoantibodi, dan memicu reaksi kaskade autoimun. Salah satu reaksi stress oksidatif adalah hipoalbuminemia. Hipoalbuminemia didefinisikan sebagai kurangnya kadar albumin intravascular dari kadar normal yakni berkisar di bawah 3,5 g/dL. Hipoalbuminemia merupakan salah satu faktor dari stress oksidatif yang dapat menyebabkan suatu kondisi autoimunitas.

 

Abstract

Pemphigus vulgaris is a disease resulting from an autoimmune disorder in the form of vesicles or bullae on the skin or mucosa originating from the suprabasal layer of the epidermis resulting from the production of autoantibodies to desmoglein 1 and 3. Usually, blisters appear on the skin that appear normal, but can develop on the skin. erythematous. Because pemphigus vulgaris blisters are fragile, the most common skin lesion observed in patients is erosion due to ruptured blisters. The diagnosis of pemphigus vulgaris can be confirmed if positive results are found on clinical examination in the form of a positive Nikolsy sign, histological examination, and immunologic test, or two signs that lead to the diagnosis of pemphigus vulgaris and the presence of an immunologic test). The management of pemphigus vulgaris is divided into 3 phases, namely the control phase, the consolidation phase, and the maintenance phase which is focused on giving prednisone corticosteroids that are given according to certain doses with close monitoring to prevent systemic side effects that can occur due to long-term corticosteroid administration. In hypertensive patients with pemphigus vulgaris, the dose of methylprednisolone given is 40-125 mg in divided doses. Pemphigus vulgaris can be caused by oxidative stress which in excess of the body's antioxidant capacity can cause cell apoptosis and accumulation of apoptotic remnants, promote autoantibody formation, and trigger autoimmune cascade reactions. One of the oxidative stress reactions is hypoalbuminemia. Hypoalbuminemia is defined as a lack of intravascular albumin levels below normal levels of less than 3.5 g/dL. Hypoalbuminemia is one of the factors of oxidative stress that can cause an autoimmunity condition.
